Prefer to Fill Out Forms at the Office?
That's fine. We ask that you arrive 15 minutes early to your first appointment to allow time to complete paperwork at the front desk. If possible, bring your insurance card and a photo ID.
What to Bring to Your First Appointment
- Completed new patient forms (if downloaded in advance)
- Dental insurance card
- Photo ID
- List of current medications, if any
- Any recent dental X-rays from a previous provider (optional but helpful)
